how is it done?
its about time some of you guys reveal some secrets for the up and coming photographers.. how do you get that smooth skin quality, with the rich texture of the cleanest, shiniest skin?

Pixel by pixel. I don't use blur filters.

Retouching skin and preserving a natural feel is not a one-click operation, and definitely not exclusively a filter technique.  Your retouching work suggests you use some sort of blur tool or rubber stamp at less than 100%, which will quickly remove all traces of natural skin.  Great skin imaging is a combination of many things:  healing and stamp tools, layer masking, snapshots and history brushing, a great deal artistry in PS, careful lighting, a good mu, and lots of patience.  I suggest a PS book or tutorial - very worthwhile investments.
