Ok, during a shoot, what was your most memorable" omg that could have turned out really bad" moment at a shoot?

This photo will explain mine. I did not notice it till after I had her climb up there and I snapped SEVERAL shots. Look at the sign.

Once I noticed the sign I about dropped my camera tryin to get to her and told her,,,,carefully,,,,,,,get on my back so I can help you off of this fence. I showed her and mom why after that and they were like um death is NOT covered in your release. Laughed it off and she got back up there to pose for this silly shot.
