first, it works best if you have an idea of how your final image should look.
then you either take apicture as starting point and paint on it
or add other pictures in other layers and mix and match.
but knowing what you're looking for helps a lot smile

Figure out what you want the image to say.
Determine what visual aspects are required for that.
Use the tools to create those visual aspects.

Without knowing what you want to say, we can't help with the visual aspects. Without the visual aspects, we can't help you with the tools.

This is an even broader question than 'how do I take a good picture'.

I think OP's question may be a little too broad.  Maybe if you posted what precisely you were trying to do you might get more direct responses, "fantasy image" covers a whole lot of ground.

I've used Bryce for my fantasy BGs then import into CS4 throw a model in there and bingo..Fantasy pic smile

You should learn in detail how to use layers in photoshop.   There are a couple of monthly magazines, one is Advanced Photoshop.  They have tutorials every issue and usually have all the necesary files on a disk which is included, or at least links to the stock photos used.  That in my opinion a good place to start.  I have done one of these, its in my portfolio.  Page three.  Its the first time I attempted something like this and I did it from one of those tutorials. 
There are many different styles.  I personally like the steam punk style.
